My Story - Why I am doing this:
I observed a provider looking for some lab results in order to make a dosage decision. The situation was urgent, if not dire. Looking through two charts, each of which were 3-4″ thick, the provider could not find the data, and I observed the look on her face change from calm to frustrated to near-panic. No provider should ever be in that position, and no patient should ever see it.
